Benjamin L. Kolly PT, DPT, OCS, OMPT, ATC

Ben worked as an Athletic Trainer for eight years prior to receiving his Physical Therapy degree. Shortly thereafter he pursued his Doctorate degree as well as a specialist degree in Orthopedic Manual Physical Therapy (OMPT).   He is a board certified Orthopeadic Clinical Specialist (OCS).  Ben has the most credentials, educational experience (13 collegiate years in the field), and hands-on training (certification in manual therapy, athletic training, and clinical hours) found in Northern Michigan Physical Therapists.

He was born in the Detroit area and raised in Petoskey Michigan. He has worked in Petoskey, Alpena, Gaylord, Rogers City, and Indian River for much of his professional career.  Since 2004 he has been working in Indian River as your Physical Therapist.

Ben is a member of the American Physical Therapy Association (APTA), Orthopaedic Physical Therapy Group, Michigan Physical Therapy Association (MPTA), National Athletic Trainers’ Association (NATA), and Michigan Athletic Trainers Society (MATS). He has taught anatomy, kinesiology, and personal health at the collegiate level.  Currently, he is faculty of Macomb Community College and teaches the orthopaedic section of their curriculum.  He has attended numerous continuing education classes and spent 13 years of college attaining his numerous degrees.  Ben also presented (lectured) about spinal rehabilitation at the national level.

Ben is certified to perform FCE's and is an APTA Credentialed Clinical Instructor for student physical therapists.  His specialties include spinal care, industrial/occupational medicine, athletic medicine, and challenging orthopedic injuries.   He has helped numerous people in Indian River with joint replacements, rotator cuff repairs, tendonitis, back and neck pain, knee surgeries to name a few as long time manager prior to opening his own facility.

In his spare time he referees collegiate and amateur soccer, instructs youth referees, and assesses referees throughout the state.  He is a member of the Knights of Columbus and active in the church.  He was elected to the Parks and Recreation Board for Tuscarora Twp in 2008.   Ben and his wife, Julie, live in Indian River with their three children, Andrew, Marissa, and Ty.

Julie has been practicing Occupational Therapy since 1997.

She is an Eastern Michigan University graduate and American Occupational Therapy Association (AOTA) member.

She was raised in Toledo, OH, but has resided in Michigan since her collegiate years.

She has worked in various settings including skilled nursing units, pediatric settings, home care, and outpatient physical therapy settings. Her skills range from fine motor activities to bracing/splinting to functional training to cognitive reeducation. She has worked with patients of all ages from young children to senior citizens.

Julie enjoys spending time with her children and scrapbooking.

Nolan has recently returned to the great north to join the Xcel family.  Nolan is returning from Hilton Head South Carolina where he was the head athletic trainer at Hilton Head Christian Academy.   Prior to his time in the south, Nolan spent three years working in the northern Michigan area and for Inland Lakes schools. 

Nolan has a bachelors of science in Athletic Training/Sports Medicine from Central Michigan University; he is a member of the National Athletic Trainers Association and the South Carolina Athletic Trainers Association.  Along with his clinical experience he has worked athletics at the high school, collegiate and professional level. 

When away from work Nolan enjoys going to the beach, boating, golf, and staying active.

Jennifer A. Andrew, Certified Athletic Trainer

Jennifer graduated from McNeese State University in Lake Charles, Louisiana with her Bachelor of Science in Athletic Training as well as her Masters of Education.  Jennifer has been working in the   physical therapy out-patient setting for  over 24 years in Lake Charles, LA , The Houston Medical Center,  Gaylord , Cheboygan and Indian River, MI.  She has also been  treating athletes since 1988 at the high school, collegiate and professional levels.  She also spent time working with the Houston Ballet.  While working the summer Olympic Track & Field Trials in 1992, she met her husband, Billy, and moved to northern Michigan in 1994.  They have 3 children, Will, Casey, and Kylee.  She strives to keep up on the latest advancements in her profession and give her patients the best quality care possible.

Jennifer Forman

Jenny has been a Physical Therapy Technician for the past 3 years.

She was raised in various places due to her father being in the armed forces, but has been in Michigan since high school.

Jenny is also a certified WaterArt Warm Water Exercise Instructor.

She has worked in various medical settings including physical therapist, optometrist and doctor offices.

Jenny and her husband Chris live in Vanderbilt with their two sons, Casey and Kyle.  In her spare time she enjoys spending time with her family while camping, boating and snowmobiling.

Lisa has previously worked in the Physical Therapy field as a receptionist and physical therapy aide.  She is certified in Adult-Child-Infant CPR, AED and First Aid.

Lisa is married to Phil and they have one adult son, Brian, and two daughters, Kari and Kalen.  Away from work, Lisa enjoys scrapbooking, rubber stamping, and weekend away with the girls working on those projects.

Lisa is excited to be part of the Xcel Physical Therapy team.
